You can choose a lot for your clinic or load one from your gallery to begin building. You can also purchase a pre-built clinic if you are starting from scratch. Make sure you have a pet treat vending machine.
Your Sim will have to buy a home lot first, then a lot for the clinic. But basically, yes, your Sim can have the clinic and the comforts of home on the same lot. Just be aware that if your Sim has a job, he or she will return to the home lot after work. Also if you click on the "home" button on your Sim's thumbnail, your Sim will go to the home

One sim I controlled and 3 other npc vets. It seems like running a vet clinic could be really profitable. But I might need to have 2 other sims crafting the serum and wellness treats while the others take care of customers. Or get all 6 sims in the household to constantly make the serum and treats until I get too bored.
So I usually open for 7-8 hours at a time, usually starting in the morning. I also control if clients can come or not so it doesn't get overcrowded and sims leaving bad reviews for long waiting time. I try to keep to a schedule of opening between 8 and 10 in the morning and closing at 4 or 5 PM for four or five days a week when i play a vet
